All services require a free scheduled intake and assessment process (30 minutes) which can be completed by phone or in person.

The initiation of services require a signed Service Agreement by the client or Medical Power of Attorney, in addition to the Durable Power of Attorney when applicable. 

​

For clients an families currently on, or about to transition to, hospice services a Release of Information is strongly encouraged in addition to the signed Service Agreement for coordination of compassionate death doula care.
